Laparoscopic cholecystectomy is one of the most common surgeries performed nowadays. There are lot of advances in closure of cystic duct and artery (clip ligation, suture ligation), but it remains an enigma regarding efficacy, safety and postoperative complications for using non-absorbable suture material or Liga clip for the operating surgeon in laparoscopic cholecystectomy. Our study aimed to evaluate the efficacy, safety and complications of non-absorbable sutures ligation versus clips application in laparoscopic cholecystectomy, and to compare the operative time and cost effectiveness of the two surgical approaches in laparoscopic cholecystectomy. Background: Present study was undertaken to compare the advantages of Clipless Laparoscopic Cholecystectomy (CLLC) over conventional laparoscopic cholecystectomy (CLC). Material & methods: This prospective randomized study was conducted in the Department of Surgery IGMC, Shimla over a period of 1 year starting from 1st july 2019 to 30th june 2020. In this study 100 consecutive patients of symptomatic cholelithiasis confirmed by USG reporting were participated and randomized into 2 groups of 50 each. Group 1 patients underwent CLC and Group 2 patients underwent CLLC. Outcomes were measured in terms of operative time, intra operative stone spillage, intra operative blood loss, amount of CO2 used , post operative pain , conversion into open cholecystectomy and post operative complications. Results: In the present study among 100 patients, 19 were males and 81 were females. Among them, 5 males and 45 females underwent CLC using electrocautery while 14 males and 36 females underwent CLLC. Mean age of males were 47.60 ± 3.36 years and females were 45.20 ± 15.82 years in CLC group and mean age of males were 53.07 ± 18.96 and females were 44.44 ± 15.41 in CLLC group. Present study showed that there was significant less Operative Time, Less Mean CO2 used, less Approx. Blood Loss, less Average pain score at 6 & 24 hours, less Average Length of hospital stay, less Intra Op. Stone spillage and less Use of Drain in CLLC as compared to CLC but there was no significant difference in Conversion to Open Cholecystectomy and Post op. Complications in both groups. Conclusion: Study concluded that CLLC had significant advantage over CLC in terms of operative time ,intra operative stone spillage , intra operative blood loss , amount of CO2 used , post operative pain & Use of Drain

Clipless Laparoscopic Cholecystectomy: Our Experience2019 •

Background: Advanced energy sources, such as the harmonic scalpel, though expensive, may provide the advantage of shorter operating time by reducing smoke, bloodless dissection in the GB bed, lower risk of bleeding from the cystic artery due to secure vessel sealing, and avoiding the use of a larger number of titanium clips. This study was undertaken to compare the advantages of Single clip laparoscopic cholecystectomy (SCLC) and Two Clip Laparoscopic Cholecystectomy (TCLC). Material & Methods: This prospective randomized study was conducted in the Department of Surgery IGMC, Shimla over a period of 1 year starting from 1st july 2019 to 30th june 2020. Mean age of males were 48.50 ± 14.49 and females were 46.73 ± 16.16 in SCLC group while mean age of males were 45.36 ± 14.53 and females were 45.25 ± 15.97 in two clips laparoscopic cholecystectomy group. Present study showed that there was more Operative Time, more Mean CO2 used, more Approx. Blood Loss, more Intra Op. Stone spillage & more Use of Drain but less Average pain score at 6 & 24 hours & less Average Length of hospital stay in DCLC group as compared to SCLC group but this difference was not statistically significant. Similarly in both group equal number of patients Convered to Open Cholecystectomy and there was no Post op. Complications in both groups. Conclusion: Study concluded that there was no statistically significant difference among both SCLC and DCLC groups in terms of operative time , intra operative stone spillage , Use of Drain, intra operative blood loss , amount of CO2 used , post operative pain, Conversion to Open Cholecystectomy & post operative complications.

Background: Securing the cystic duct and artery is the most important step in laparoscopic cholecystectomy. Several techniques are followed for the same, such as clips, intra or extra corporeal ligation, harmonic scalpel, LigaSure etc. Intracorporeal ligation has the advantage of using standard knotting method, without the need for extracorporeal knot creation, pushing device, accessing appropriate tension without the risk of cutting through the tissue and the ability to secure knot in difficult locations. Single ligation of cystic artery and duct (SLAD), as No clip laparoscopic cholecystectomy (NCLC) is time saving, as both the duct and artery are secured in a single ligature. Methods: Since September 2018 to February 2019, we used intracorporeal single ligation of cystic artery and duct in 40 patients with symptomatic Gall bladder stones undergoing Laparoscopic cholecystectomy (LC). Patients were discharged on 1st or 2nd postoperative day and followed up in our outpatient department on 5th or 6th postoperative day. The 2nd outpatient visit was 8-10 days later for suture removal and to review histopathology report. Results: We performed SLAD in 40 patients with symptomatic Gall bladder stones. Patients were in the age group of 17-60 yrs. Average age was 45yrs. There was a slight Female predominance with 24 out of 40 (60%). We were able to tie the cystic duct and artery in single ligature. We had no bleeding complications. The mean time taken for ligation was 3.5min.. Conclusions: Single ligation of cystic artery and duct is feasible and safe alternative method to secure cystic artery and duct in laparoscopic cholecystectomy. Although it can be used routinely, it gives an added advantage over clips in cases with thick oedematous cystic artery and duct complex. There is no increase in operating time as only one ligature is used.

A 6-year Prospective Clinical Study of Laparoscopic Cholecystectomies in Kashmir Valley, IndiaToday Laparoscopic cholecystectomy has replaced open cholecystectomy as the 'gold standard' in treatment of patients with symptomatic cholelithiasis. Advantages being relatively less pain, early ambulation, shorter hospital stay and lower incidence of incisional hernia. The condition of the patient, the level of experience of the surgeon, and technical factors all can play a role in the decision for conversion. This study was conducted in an effort to determine the conversion rate and also identify the factors responsible for conversion of laparoscopic cholecystectomy to open cholecystectomy. Hence, these findings will allow us to preoperatively discuss the higher risk of conversion and allow for an earlier judgement and decision on conversion if intra-operative difficulty is encountered. This was a prospective clinical study consisting of 112 patients undergoinglaparoscopic cholecystectomy. All patients were admitted in Bankura Sammilani Medical College and Hospital from June 2014 to May 2015. Data was collected by meticulous history taking, careful clinical examination, appropriate radiological, haematological investigation, operative findings and follow-up of the cases. In conclusion, laparoscopic cholecystectomy is a safe and minimally invasive technique, with only low conversion rate and the commonest cause of conversion in this study was the presence of dense adhesions at Calot's triangle

Objective: A laparoscopic cholecystectomy (LC) is the treatment of choice for gallbladder diseases. The aim of this study is to analyze laparoscopic cholecystectomies performed by a single surgeon over 8 years at Nobel Medical College Teaching Hospital and Research Centre Pvt. Ltd. of Biratnagar, Nepal (NMCTH). Methods: We performed a retrospective analysis of 7557 patients that underwent a LC by a single surgeon, from October 2010 to July 2018. We divided it into 3 groups that include data of every three years. Results: The hospital stay, operation time and conversion rates were decreased by years of experience. Female patients outnumbered male (M:F=1:3.7) patients in this study. The hospital stay (3 days), operation time (21 min), conversion (0.3%) and complications (1.9%) rates decreased from Group 1 to Group 3 (p<0.05).
